Children's Health Home

Yeled v’Yalda’s Health Home program connects families with a network of providers, helping manage healthcare needs through a dedicated care manager who coordinates services and access to resources.

Home & Community Based Services (HCBS)

Home and Community Based Services (HCBS) provide Medicaid-funded support for medically fragile children or those at risk, offering care at home and in the community to avoid more restrictive settings.

Caregiver Family Advocacy Supports Services (CFASS)

Caregiver/Family Advocacy and Support Services equip families with essential tools to help children with disabilities thrive within their homes and communities. By offering education, resources, and guidance, these services empower caregivers to address unique needs and strengthen family support systems.

Respite Services

Respite care offers short-term relief for caregivers of children with disabilities, ensuring safe supervision and engagement to support the child’s health and development. Provided by trained staff, care can take place at home, in community settings, or other approved locations.

Pre-vocational
Services

Preparing youth for the workforce by teaching job readiness skills and introducing them to workplace expectations.

Community Habilitation Services

Helping youth develop and enhance daily living skills. This includes activities that foster self-care, safety, communication, problem-solving, and social interaction.

Yeled v'Yalda’s Early Intervention program supports children (0-3) in reaching developmental milestones, providing timely help to address delays and promote healthy growth.

Therapy Referrals

Yeled v'Yalda offers school-based counseling, collaborating with families and teachers to support students' emotional development, help them overcome challenges, and reach their goals.

SEIT Referrals

Yeled v’Yalda creates inclusive learning environments tailored to each child’s needs. Their SEIT program helps preschoolers (ages 3-5) develop school-readiness skills, fostering future academic success.
